City Security Access Cards

New Requests:
The Security Access Card Form is the form that needs to be filled out for the initial issue of a security card, regardless of where the employee will be working. The form is color-coded and the Blue sections need to be filled out by the requesting Supervisor or their designee. The form will then be e-mailed to [email protected].

The Access security card will be issued and that portion of the form will be completed the Security office. They will then forward the completed form back to the requesting Supervisor who will have the employee sign it and it will be maintained in their employee file. Mark Smith will either deliver the card or let the requestor know where it can be picked up. THE CARDS WILL NOT BE SENT VIA INTER-OFFICE MAIL. The Security office will file an electronic copy of the completed request form.

Change Request:
The Access Change Form should be used to change an existing cardholder's access level. It is color-coded and the Blue sections need to be filled out by the requesting Supervisor. The supervisor will then e-mail it to [email protected].

An email will be sent to the requesting supervisor when the change has been made. When completed, the Security Office will file the form electronically.

Fitness Room Access Cards:
The Fitness Room Access Card Form is to allow city employees access to the fitness rooms at the Public Safety Facilities. An employee asking for access shall complete this form and e-mail it to [email protected]. If a new card needs to be issued, the employee will be required to pay a $5.00 fee, which is non-refundable. If the employee already has a security card issued the Security office will ascertain the information necessary to activate that card, there is no charge to the employee. Once the request is completed the employee will be notified by email for existing cards and called for new cards. When the employee is called the Security office will inform the employee where the card can be picked and where the fee needs to be paid. Upon leaving employment of the city of Glendale, these cards will be deactivated by the Security office, upon notification by Human Resources & Risk Management, and need to be turned in to their supervisor prior to departure.



All security cards must be returned to the Security office when an employee is no longer employed. A department cannot merely keep the card to give to a new employee.
