WHAT: Benefits eligible employees have the opportunity to earn annual Savings - up to $360 - on the City's Blue Cross Blue Shield of Arizona health plan.

• Earn $120 at Level 2, another $120 at Level 3, and an additional $120 at Level 4! Annual savings up to $360 if you reach Level 4.
• BONUS CASH - All benefits eligible employees can earn $100 in rewards (Pulse Cash) by reaching Level 4 in the WellStyles portal. 
  

WHEN: The earning period is: July 1, 2024 - June 15, 2025. The wellness incentive savings begins the first pay in July 2024 and goes throughout the year.

HOW: Building healthy habits earns points towards your incentive. Track activities in the WellStyles portal; available to all benefits eligible employees whether or not you are on the BCBSAZ plan.
